Foster Kennedy syndrome: papilledema in one eye with optic atrophy in the other eye.

gency department because of gradual blurring of vision in his right eye over 20 days and progressive loss of vision in the left eye over the previous eight months. One year earlier, he had started taking sodium valproate for mor ning migraines with only a partial response. Visual acuity was 20/30 (66.7%) in the right eye, and he could perceive only hand motions with the left eye. He also had a left relative afferent pupillary defect. Funduscopy showed a pale left optic disk and edema of the right optic disk (Figure 1). The other results of neurologic examination were normal. These findings suggested Foster Kennedy syndrome, in which an anterior intracranial mass directly compresses the ipsilateral optic nerve, causing atrophy, and increases intracranial pressure, which results in contralateral papilledema. Cranial computed tomography showed a mass arising from the left anterior clinoid process (see Appendix 1, available at www .cmaj .ca /lookup /suppl /doi:10.1503 /cmaj .101937/-/DC1). Systemic dexa methasone and phenytoin were started prophylactically, and the patient was admitted for surgical excision of the mass. At follow-up one year after the surgery, visual acuity in the right eye had improved to 20/20 (100%), and the patient was able to count fingers with his left eye. Foster Kennedy syndrome is thought to be present in 1% to 2.5% of intracranial masses. Tumours of the frontal lobe can become quite large before they are discovered, and focal symptoms (e.g., memory loss or emotional lability) may be sparse. Although the increase in intra cranial pressure correlates with the size and location of the tumour, related symptoms may be subtle and less prominent than pupillary changes, if the elevation of pressure is longstanding. Recovery of visual acuity may occur with normalization of the intracranial pressure. This case highlights that a diagnosis of migraine should be questioned when the headache is atypical, is only partially controlled by medications or is associated with other neurologic symptoms.

An Aggressive Sphenoid Wing Meningioma Causing Foster Kennedy Syndrome
Foster Kennedy syndrome is a rare neurological condition with ophthalmic significance that can manifest as acute visual loss. It is classically characterised by unilateral optic nerve atrophy and contralateral papilledema resulting from an intracranial neoplasm. Physicians should consider Foster Kennedy syndrome in patients who present with visual loss and who have a history of intracranial neo...
متن کاملPseudo-Foster Kennedy Syndrome due to unilateral optic nerve hypoplasia: a case report
INTRODUCTION Pseudo-Foster Kennedy Syndrome is described as unilateral optic disc swelling with contralateral optic atrophy in the absence of an intracranial mass causing compression of the optic nerve. This occurs typically due to bilateral sequential optic neuritis or ischaemic optic neuropathy. CASE PRESENTATION We describe a case of pseudo-Foster Kennedy Syndrome in a two year old boy wit...
متن کاملPseudo-Foster Kennedy Syndrome as a Rare Presentation of Vitamin B12 Deficiency
INTRODUCTION Pseudo-Foster Kennedy syndrome is a triad consisting of ipsilateral optic atrophy, contralateral optic disc edema, and ipsilateral anosmia in the absence of an intracranial mass. Vitamin B12 plays an important role in DNA synthesis, and its deficiency causes peripheral neuropathy, myeloneuropathy, and, very rarely, optic neuropathy. CASE PRESENTATION In this study, we describe a ...

Tumors of the olfactory groove may cause unilateral optic atrophy with contralateral papilledema and anosmia (Foster Kennedy syndrome). We describe a case of a young pregnant woman with Foster Kennedy syndrome due to an olfactory groove meningioma.

Objective: To report a case of Pseudo-Foster Kennedy (PFK) syndrome and describe its clinical and paraclinical particularities, as well as the diagnostic difficulties and established treatment. Methods: The case of a 60-year-old male patient with sudden, painless visual impairment in the left eye (LE), and a medical history of old optic nerve atrophy in his right eye (RE) was described. Results...
